Individuals ages 5 and up, receiving their first COVID-19 vaccine dose, will receive a $100 gift card.
Walk-in, or pre-register by calling 216-664-2222 between 9am and 5pm, or visit https://gettheshot.coronavirus.ohio.gov. CDPH staff will be administering Pfizer, Moderna, and Johnson & Johnson vaccines. Anyone younger than 18 must be accompanied by a parent or legal guardian to obtain the vaccine.
